Care Act assessment

Visit notes written up against the eligibility outcomes, with the person’s own goal leading.

Social worker: Home visit notes: older adult living alone, struggling with the stairs and with meals, daughter visits at weekends, wants to stay at home. Write up the assessment. Social Care: Two of the ten specified outcomes are engaged, which meets the threshold of two or more. Eligibility also requires needs arising from a physical or mental impairment or illness, and a significant impact on wellbeing as a result. Questions

What happens to what we send?

It is used to answer your request and nothing else. It is never used to train, tune or improve the model. It is encrypted in transit and at rest, and every request and answer is recorded so you can see what was asked and what came back.

Where does the model run?

In the UK. The Social Care model is built, hosted and maintained in the UK by the Institute of AI.

Which guidance does it draw on?

The Care Act and its statutory guidance, the Children Act, Working Together to Safeguard Children, and the local thresholds you provide. Every answer names the duty or guidance it rests on so it can be checked.

How do we access the model?

Access is by request. Tell the Institute of AI which authority or provider you work for and what you would use the model for. The Social Care model is available through a secure chat workspace, an API or agents.
