Discharge summary
Ward notes go in and a structured summary comes out, in the trust’s format and ready for sign-off.
Ward doctor: Ward notes: 72F, adm 4/3 with LRTI, started PO abx D1, obs stable since, 5/7 course not yet complete, home today. Draft the discharge summary. Health Service: Diagnosis: lower respiratory tract infection. Treatment given: oral antibiotics started on the day of admission, with observations stable since. Medicines on discharge: a five day course of oral antibiotics, not yet complete, to be finished at home as prescribed. Follow-up and safety-netting: not covered by the ward notes. What happens to what we send?
It is used to answer your request and nothing else. It is never used to train, tune or improve the model. It is encrypted in transit and at rest, and every request and answer is recorded so you can see what was asked and what came back.
Where does the model run?
In the UK. The Health Service model is built, hosted and maintained in the UK by the Institute of AI.
Which guidance does it draw on?
Published UK clinical guidance, record standards such as the PRSB eDischarge Summary Standard, and UK clinical terminology. Every answer names the guidance or standard it rests on so it can be checked.
How do we access the model?
Access is by request. Tell the Institute of AI which trust or practice you work for and what you would use the model for. The Health Service model is available through a secure chat workspace, an API or agents.
